This is the same secure process used when a practice replies snapshot to a KATYCS invitation. There is no second medical questionnaire on this website.

  1. Reply snapshot

    KATYCS confirms the request before issuing access.

  2. Receive temporary practice access

    The practice receives the secure portal link and its time limited access code.

  3. Complete the Capacity Snapshot

    The existing portal provides the questionnaire and approved non-PII templates.

  4. Discuss the findings

    KATYCS explains the review and discusses a suitable next step on a call.
